The standard installation path for TL'09 is
%SystemDrive%/texlive/2009, so no spaces there. Did you have it
differently?

Shouldn't matter. We use wrappers to give precedence to our Perl.

DEP could be the problem given the 0xc0000005 error (access
violation). Worth checking.
