dvitodvi crashes on documents with >1000 pages

Hi All,

The program `dvitodvi` when applied to a document with >1000 pages,
crashes due to a buffer overflow on a fixed size page lookup table. The
same problem happens with `dvibook`.

I have a patch that probably fixes the issue (see attachment); but do
not know where to submit it for review. Is there a guide for one-off
contributions like this?
