Details:

There is spurious "]" in the output document when package hyperref is used
with tex4ht in new texlive 2013. This issue was reported at 

http://tex.stackexchange.com/q/120617/2891

Heiko Oberdiek posted some patch, which I have adopted for literate tex4ht
sources, the patch is included in the attachment. As I don't have texlive 2013
yet, I cannot test it, but I hope it is correct.
