[texhax] Error `misplaced alignment tab and character

When running the command pdflatex file.tex 
I'm getting the error

! Misplaced alignment tab and character

Can someone point me to a ref on how to fix this? I can't determine any
errors in either the bib or tex files. What should I look for?
