You mean that you want to create a regular index and a formulas index, 
right? Once I used the index package by David M. Jones:

It's ``old'' by today's standards, by I found it very effective. If you 
have TeXLive 2011 installed, you can find it's documentation in

/usr/local/texlive/2011/texmf-dist/doc/latex/index/
